Names provided in this form will be added to the list that we print in our weekly Shabbat bulletin.

We are only accepting names of BDJ members and their relatives. 

You may keep the name anonymous by submitting the Hebrew name only, or include identifying information. You will have the option to include the English name and/or relationship to yourself or another BDJ member.
